Complete a Brief Questionnaire
As enrollment approaches, you will receive a short questionnaire about your health priorities and what you are looking for in a physician. This helps us determine if the practice is a good fit for your needs.
Talk with Your Doctor
If the questionnaire suggests a good match, we will invite you to a brief introductory call so you can meet your physician, ask questions, and decide if the practice is right for you.
Enroll and Build Your Baseline
Accepted members begin the Foundation Period. Over the first several months, we build a thorough understanding of your health through intake, labs, and a comprehensive first visit. This is how your physician gets to know you deeply.
Care That Builds Over Time
After your baseline is established, your care settles into a steady rhythm. Regular visits, secure messaging, and a physician who knows more about you with every conversation.
